I think that was the game winning play on The Longest Yard (Sandler version) too, wasn't it?
As to the Line Left/Right play, my alma mater sets up in that on EVERY extra point. Been doing it for nearly 20 years now, and I've seen it work twice. Normally, the defense simply lines up to match the kicking team's alignment, the line adjusts back to the standard kicking set-up and they kick the extra point.
However, on two occasions, the defense didn't. Once, the defense lined up as normal instead of adjusting. The center snapped to the holder who quickly threw the lateral to the HB behind the line at the hash mark who walked in for a two-pt. conversion. Another time, the defense matched the line on the hash, but left only one man over center. The center snapped, and the holder ran in untouched.
