A Ram Fake FG That DID Work (some trivia)

Here's the transcript of the radio call on a Rams' fake FG that did work. Fill in the blanks if you remember this play:

..."Rams setting up for the field goal that would win them the NFC west division and beat [OPPOSING TEAM] here today in overtime at the [STADIUM]......[HOLDER] sets and [KICKER] lines up.....he will be kicking from East to west uprights..it'll be about a twenty yard kick...the line sets....[KICKER] signals......there's the snap....[HOLDER] places it...NO, it's a fake !!!!....[HOLDER] races for the southwest corner...TOUCHDOWN RAMS TOUCHDOWN RAMS!!!!!!! RAMS WIN 27 - 21 IN OT !!!!

[HOLDER] is being mobbed in the back of the endzone by teammates, WOW what a finish..The Rams Win the NFC West...This one is history, Rams beat [OPPOSING TEAM] and are poised for the playoffs !!!!

Young [QB] the Start and an old [OPPOSING TEAM] [PLAYER] comes in and finishes the [OPPOSING TEAM] jinx off !! These [YEAR] RAMS are exciting!"

And, for a bonus - which Ram player was credited with suggesting this play?

Re: A Ram Fake FG That DID Work (some trivia)

GC got most of it! RB got the kicker.

..."Rams setting up for the field goal that would win them the NFC west division and beat Minnesota here today in overtime at the Collesium......Cromwell sets and Corral lines up.....he will be kicking from East to west uprights..it'll be about a twenty yard kick...the line sets....Corral signals.....there's the snap....Cromwell places it...NO, it's a fake !!!!....Cromwell races for the southwest corner...TOUCHDOWN RAMS TOUCHDOWN RAMS!!!!!!! Cromwell is being mobbed in the back of the endzone by teammates, WOW what a finish..The Rams Win the NFC West...This one is history, Rams beat Minnesota and are poised for the playoffs !!!!

Young [QB] gets the Start and an old Viking [PLAYER] comes in and finishes the Viking jinx off !! These 1979 RAMS are exciting!"

    So...

Who were the two QBs who played for the Rams (one being a former Viking)?

    And...

    Who suggested the play?
